Kernodle Clinic will be conducting free sports physicals on our campus Saturday, July 20th from 2:00pm to 4:00pm for any student who needs a physical for the upcoming sports season. We will be using the world language building since the gym is under construction.
They asked that we extend the invitation to our feeder middle school as well.

The sports physical consent form that the kids need to have completed by a parent in order to get physicals completed. They are also asking that students have pages 1 & 2 (Health History Forms) of the sports physical packet completed prior to their physical. Please let me know if you have any questions.
